How To Gather Information From Clients Using Forms

Gathering information from clients just got easier with Fidu forms!

Here's how to gather information from clients using a form:

1. Navigate to the Fidu Menu: Head over to the "Productize" section on your dashboard.

2. Create a New Form: Click "New" and select "Form."

3. Give it a Title: Choose a clear name that reflects the form's purpose (e.g., "Trademark Owner Information"), you can always update this information at a later time.

4. Categorize it Clearly: Select the appropriate topic category (e.g., "Trademark").

5. Start with Sections: Before writing your first question, you will need to create a Section.

7. Add questions: There are a variety of question formats you can use within forms:

  • Text Field: This is best for simple, text-based questions or numbers you do not want formated (like a phone number).

  • Multi-line Text: Choose this if you need your client to provide a detailed response.

  • Date: If you need your client to provide a date, use this format.

  • Number: This is ideal for questions requiring a numerical answer.

  • Radio Button: Use this if you want your client to select one option from multiple choices.

  • Checkbox: This allows your client to choose multiple responses.

  • File: If you need your client to upload a file, choose this option.

8. Make it Mandatory (Optional): Use the "Required" option to ensure clients fill in essential questions. You can turn this off by unchecking the box if you do not need an answer to the specific question.

9. Add Help Text: Provide additional context or instructions for complex questions using the "Help Text" feature. It will be shown underneath the question.

You can add multiple sections to any form if needed.

As a Productize resource, your form can be shared with clients via a custom Page or Project.
